To comply with state law, please complete the 2025–26 Health Services Annual Parent/Guardian Consent Form to indicate your decision. This ensures we provide only the services you authorize.

parent/guardian consent before providing classroom guidance lessons and well-being questionnaires.

your student’s well-being and academic success.
